Warranty Registration
Thank you for choosing Beyond Med Shop. Please note that all Zacurate®, products are covered by a 1-year warranty when purchased from an authorized dealer. If you purchased a Zacurate Premium 500E Pulse Oximeter, your warranty coverage is valid for 18 months. Authorized dealers include: “Amazon.com”, “Walmart.com” and “Beyond Med Shop”.
Please do not submit a warranty registration form to file a warranty claim. You may use the form only to register your product. A warranty registration submission is not the proper way to file a warranty claim.
To file a warranty claim please send an email to cs@beyondmedshop.com with the subject line ‘Warranty Claim’ and include the following:
Your Name
Your Phone Number
Item Description
Summary of Issues
A copy of your Proof of Purchase (Receipt / Sales Order)
Acceptable Proof of Purchase Submissions include:
- PDF or Word Document of purchase receipt
- Screenshot / Print-screen of purchase receipt (Mobile or Desktop)
- Any other document or file containing:
- Order Date
- Order ID
- Item Number / Description
Warranty claims will not be honored if products were purchased from unauthorized dealers.
